POSITION DESCRIPTION:

In alignment with the Mission, Vision and Values of Haliburton Highlands Health Services, the Admission Avoidance Lead will provide part-time support to the emergency department and acute care to identify and assess medically stable patients who would benefit from increased community supports to avoid admission and return visits to the Emergency Department.

In this role, the Admission Avoidance Lead, will be an autonomous allied health professional who works in partnership with clients and others, such as family members and community partners, to provide safe, effective, ethical and client centered care. This position will help clients over 65 who have limited abilities to establish, develop and/or maintain the ability to complete daily functions and tasks. As a member of the interprofessional team the Admission Avoidance Lead will provide assessment, treatment recommendation and community referrals to patients seen in the Emergency Department to help support continued independence in the community and aging in place.

In Acute Care, the Admission Avoidance Lead works alongside the interprofessional team to provide assessment, intervention, education and discharge planning of referred clients in the areas of self-care, productivity and leisure. Experience with geriatric patients is an asset to this role.

QUALIFICATIONS:

  • Allied Health Professional (OT/PT/SW)
  • In good standing with their applicable College 
  • Knowledge and history of collaboration with community resources and supports 
  • Clinical experience working with geriatric patients and in hospital is an asset 
  • Strong understanding of patient flow and health system navigation 
  • Ability to organize and prioritize complex patient needs in real time 
  • Ability to work independently and collaboratively with a team 
  • Excellent interpersonal skills
