Our team at the Diabetes Education Network works together with you, your doctor and other health care providers to help you better manage all aspects of your diabetes. Our goal is to help you gain knowledge about your diabetes, advocate for your health and help develop the skills to self-manage your blood sugars.

Who are we?
We are a small team consisting of Registered Dietitians, Registered Nurses, and clerical support.
Who do we serve?
Residents of Haliburton County, 18 years of age or older with:
- Type 1 Diabetes
- Type 2 Diabetes
- Pre-Diabetes
- Gestational diabetes
- Newly diagnosed with diabetes or pre-existing diabetes
- In-patients with any of the above at HHHS acute care unit

What do we offer?
We are currently offering one-on-one appointments with our clinical team.
We provide education on all aspects of diabetes management including pharmacotherapy and lifestyle. Our goal is to empower our clients to self-manage; we will work with you to create individualized goals related to your diabetes management and reduce the risk of complications associated with unmanaged blood sugars.

Yes! Self-referrals are always welcome, you can contact our clinic via phone at: 705-286-2140 Ext. 3296 or email at: diabetes@hhhs.ca to make a referral. Please include your name and contact information when making a referral. We require consent from the person with diabetes in order for a family member to make a referral on their behalf.
What if I don’t have a family doctor?
Yes, we take clients who do not have a family doctor as well as those with family doctors who are not local.
Can I bring a support person to my appointment?
Yes, a support person is welcome to attend your appointment but please limit to one support person for space purposes.
What should I bring to my appointment?
- An updated list of medications
- Eye glasses and hearing aids if needed
- Glucometer and logbook if testing blood sugars
